Offline imports need three things
An offline conversion import needs a valid identifier, a conversion time and a target conversion action. If any of these is missing, wrong or mismatched, the import fails silently. Google does not always tell you why a row did not import, so you have to check each piece yourself.

Upload format and data formatting
The upload has to match the expected format. Column names, date formats and value types all have to be correct. A wrong column name or a date in the wrong format causes the whole row to fail. Validate your file against the required schema before uploading.
Duplicate uploads
If you upload the same conversion twice, the second upload may be rejected or may create a duplicate depending on the setup. Use a consistent identifier or transaction ID so the system can deduplicate. Track what you have already uploaded to avoid resending the same rows.
Matching
