A form conversion has to pass through several stages. The user submits the form, the form succeeds, a tracking event fires, the tag sends the data and the Google Ads conversion action records it. If any stage fails, the conversion never appears. Trace the journey to find where it stops.

Form submission vs button click
The most common mistake is firing the tag on the button click instead of the successful submission. A click does not mean the form succeeded. The event should fire on the success state, either a thank you page load or a confirmed form success event. If the tag fires on click, every failed submission looks like a conversion or none record because the success event never fires.
Thank you page vs form success event
There are two reliable ways to track a form. One is a thank you page that loads after submission, where the tag fires on page load. The other is an inline success event fired by the form on submission. Both work. The problem comes when the thank you page never loads because of a redirect, or the success event never fires because the form errors silently. Confirm which method you use and that it actually completes.
GTM trigger configuration
If you use Google Tag Manager, the trigger has to match the actual event. A trigger set to a button click will fire on click. A trigger set to a form submission may fire on submit even when validation fails.
