When automation does not update lead status after a sales action, the activity happened but the workflow never received a signal from it. The difference between an action and a trigger

A sales action and an automation trigger are not the same thing. A rep can call a lead, book an appointment or update a field without the automation ever knowing. For the status to update automatically, the sales action has to produce an event the automation can trigger on. If no event was generated, the workflow has nothing to react to. The first question is whether the trigger fired at all, not whether the status update logic is correct.

A sales rep may have manually set the status, and a later automation may have overwritten it back or left it unchanged. If the rep and the automation both update the same field, the last write wins.
